A current or former Department of Corrections (DOC) inmate may authorize the release of records pertaining to himself or herself by submitting, in person, a DOC-LD Form 1 (PDF) (or a Form 1A (PDF) if he or she is physically unable to complete a Form 1) to the Bucks County Correctional Facility Records Department. Valid photo identification will be required. Only subpoenas can be submitted by mail.
Where applicable, the DOC will include in its response to a records requests a completed DOC-LD Form 2 (PDF) and/or Form 3 (PDF).
