Children's Case Management Services

Blended Case Management (BCM)

BCM is funded through Medicaid.   BCM is non-clinical and works with the child and family to link to needed resources.   This service is community based and will partner with other services and supports that are working with the child and their family.

More information on BCM and providers can be found in Magellan’s provider directory:  https://www.magellanofpa.com/for-members/find-a-provider/provider-directories/
